The newborn is not a miniature adult. Its physiological systems are indeed still developing. However, the newborn child is able to feel the pain and probably in a more acute way tha an older child. This pain must be taken care of by health professionals. Actually, if this pain is not treated, it may bring about negative consequences in the short and in the long term, among which hypersensitivity in particular. It’s a difficult task for caregivers to assess small children’s pain as they cannot express themselves or undoubtedly demonstrate it. Several tools have been developed to help evaluate the intensity of the pain felt. A stepwise approach is recommended by international guidelines. It is based on scientific data whose levels of evidence are sometimes variable. This work aims at summing up the available treatments and their indications. The pharmacist and more specifically the hospital pharmacist, has a role to play in optimizing the care of these newborns. Le nouveau-né n'est pas un adulte en miniature, ses systèmes physiologiques ne sont pas encore tous arrivés à maturité. Il est cependant capable de ressentir la douleur et sans doute de manière plus intense qu'un enfant plus âgé.Cette douleur doit être prise en charge par les professionnels de santé. En effet, si elle n'est pas traitée, elle peut avoir de conséquences néfastes à court et à long terme dont notamment une hypersensibilité.La difficulté pour les soignants est d'objectiver la douleur chez ces enfants qui ne peuvent s'exprimer ou le démontrer de manière indubitable. Plusieurs outils ont été développés pour essayer d'évaluer l'intensité de la douleur ressentie. Une approche de prise en charge par étapes est préconisée par des recommandations internationales. Elle se base sur des données scientifiques dont le niveau d'évidence est parfois variable. Ce mémoire a pour objectif de faire le point sur les différents traitements disponibles et leurs indications. Le pharmacien, essentiellement hospitalier, a un rôle à jouer dans l'optimalisation de la prise en charge de ces nouveau-nés.

Preterm birth and hypotrophic birth is a major issue of the 21st century. It is responsible for 14.1% of deaths of children younger than five years. Therefore, it is important to further investigate different pathologies, which could lead to preterm birth. Periodontal disease has been linked to preterm birth since Offenbacher first published his study Periodontal infection as a possible risk factor for preterm and low birth weight in 1996. Many studies about the same subject have been published since then but none of them tried to see if the rate of preterm and hypotrophic birth rise as the periodontal status worsens. We propose a study, where the DPSI is used to classify the subjects in four different groups according to their periodontal status to be able to compare if the percentage of preterm and hypotrophic children rises as the periodontal status worsens.Material and Methods: Ethical approval has been granted by the Comité d'Ethique Hospitalo-Facultaire from Cliniques Universitaires Saint-Luc. Eligible subjects were asked to sign a consent form. Then a DPSI was scored on the eligible subjects between their 9th and 2llh week of pregnancy. Approximately at the expected date of labour, the medical record was checked to see if there was preterm birth or if the child was born small for gestational age (SGA).

Since 1990, the number of deaths or children under the age of 5 years has almost halved but it still remains important. In 2015, it was estimated at 5,9 million per year. The neonatal mortality represents nearly one million deaths a year. The latter is mainly due to brain damage including hypoxicischemic encephalopathy for which no treatment has yet been found for the premature babies. Erythropoietin is a molecule that as an effect on erythropoiesis and is administered to premature to prevent or treat anemia for many years. It was recently discovered that she had a direct effect on nerve cells which makes it interesting as a neuroprotective agent. It could avoid the complications that can occur in premature infants. Many studies have been made and are ongoing to evaluate its safety, its efficacy and the dose to administer. The dissertation together different informations obtained up to now and discusses the possible future of this compound as a neuroprotector. La mortalité néonatale quant à elle représente un million de décès annuels. Cette dernière est principalement due aux lésions cérébrales dont l'encéphalopathie hypoxique-ischémique pour laquelle aucun traitement n'a encore été trouvé en ce qui concerne les prématurés.L'érythropoïétine est une molécule qui a une action sur l'érythropoïèse et elle est administrée depuis de nombreuses années aux prématurés afin de prévenir ou de traiter l'anémie. Il a été récemment découvert qu'elle avait des effets directs sur les cellules nerveuses ce qui la rend intéressante en tant que neuroprotecteur. Elle pourrait ainsi éviter les nombreuses complications qui peuvent survenir suite à une naissance avant terme. De nombreuses études ont été faites et sont actuellement en cours afin d'évaluer sa sécurité, son efficacité ainsi que les doses à administrer. Ce mémoire rassemble différentes informations obtenues jusqu'à maintenant et discute d'un avenir possible de cette molécule dans cette indication.

L'incidence du syndrome de mort subite du nourrisson (MSN) a considérablement diminué ces dernières années grâce à une série de mesures et d'actions. Celles-ci ne parviennent cependant pas à éviter totalement que des enfants de moins d'un an décèdent de manière inopinée en raison de l'étiologie multifactorielle de ce syndrome. Dès les années 80, des médicaments ont été identifiés comme responsables ou potentiellement responsables de mort subite. Des spécialités ont été ainsi retirées du marché ; d'autres se sont vues imposer des restrictions d'âge et d'indication ; chez d'autres enfin, la composition a été revue. Le but de ce mémoire est d'analyser les principaux médicaments, au sein de huit classes préétablies, capables d'induire une MSN et d'explorer les rôles que les pharmaciens et les médecins ont à jouer en ce qui concerne l'administration de médicaments aux nourrissons. Les recherches effectuées mettent en lumière les principaux médicaments administrés à des nourrissons à l'heure actuelle, médicaments qui pourraient, par leurs mécanismes d'action et/ou une pharmacologie particulière en bas âge, jouer un rôle dans ce syndrome. Pour certains médicaments, la relation de cause à effet est évidente ou quasi évidente ; c'est le cas par exemple du Cisapride et de la Codéine. Pour d'autres, la littérature scientifique manque de preuves certaines, en raison de faibles séries ou de case-report ; la Dompéridone, les macrolides et les anti-malariques se retrouvent notamment dans cette catégorie. Des recherches doivent être poursuivies afin de diminuer la part que peuvent jouer les médicaments dans le syndrome de MSN.En ce qui concerne le rôle des professionnels de la santé, le médecin doit être particulièrement attentif lorsqu'il prescrit un médicament potentiellement à risque et prendre les mesures de surveillance qui s'imposent, à domicile ou à l'hôpital. Le pharmacien doit aider le médecin en discutant éventuellement de l'indication du médicament, en lui apportant ses connaissances de pharmacologie et en s'interrogeant sur les interactions médicamenteuses potentielles. Il doit aussi rappeler les signaux d'appel de malaise grave du nourrisson qui justifient que l'enfant soit vu en urgence par un médecin. The incidence of sudden infant death syndrome (SIDS) has dropped significantly in recent years due to a series of measures and actions; These measures and actions however are not sufficient to completely avoid that children under one year old die unexpectedly due to the multifactorial etiology of this syndrome; In the 80s, drugs were identified as responsible or potentially responsible for sudden death. Consequently, certain pharmaceutical specialties were withdrawn from the market; others were applied age and indication restrictions; in some cases, the composition was reviewed. The purpose of this thesis is to analyze the main drugs, within the eight previously established classes, capable of inducing SIDS and explore the roles pharmacists and doctors have to play when it comes to administering drugs to infants. The research conducted brings to light the main drugs given to infants at present; drugs that could, by their mechanisms of action and/or the particular pharmacology of toddlers, play a role in this syndrome. For some drugs, the causal relationship is obvious or almost clear; such a Cisapride and Codeine. For others, scientific literature shows lack of evidence due to low series or case-reports; Domperidone, macrolides and antimalarial agents namely reside in this category. More research needs to be done in order to reduce the part played by drugs in SIDS. With regards to the roles of the healthcare professionals, doctors should be particularly careful when prescribing a drug which could potentially be at risk for an infant and should take the necessary supervisory measures at homes or in a hospital. The pharmacists must aid the doctor, possibly by discussing the drug’s indication, by sharing his knowledge in pharmacology and investigating the potential drug interactions. He must also underline the preemptive signals which could indicate a severe disorder for the infant, in which case the child would need to be urgently brought to a doctor.

This chapter distinguishes between essential features of the zone of parental discretion and the longstanding concept of a grey zone in neonatal treatment decision-making. The grey zone has traditionally described a gestational age range where the outcomes of medical treatment for newborn infants are uncertain, and therefore parents have discretion to choose between resuscitation or palliative care options. In contrast, the ZPD refers to a space where parents may make decisions for their child (not restricted to newborns) even if their decisions conflict with the decisions a clinician would make. A key difference between the two zones is that the boundaries of the grey zone are defined on the basis of published evidence about medical outcomes, whereas the boundaries of the ZPD are based on the broader but arguably vaguer notion of harm to the particular child. The grey zone has usually been defined in terms of gestational age. Wilkinson argues instead for a prognosis-based grey zone in neonatal treatment decision-making, which incorporates a range of prognostic factors rather than focusing solely on gestational age.
